Withdrawal Card Request
As a member, you are responsible for informing the Local of ANY separation from your carrier. This is not limited to, but may include a Leave of Absence (LOA, PLOA, FMLA, CLOA, Furlough or Medical). In such a circumstance you may choose to: (1) Remain an ACTIVE member by continuing to pay the minimum dues rate of 1.56% of the lowest minimum guarantee multiplied by the lowest pay rate currently in effect under the CBA (EXAMPLE: 1.56% × CBA lowest min guarantee (70 hours) × lowest pay rate ($57.67)) OR; (2) Request a withdrawal card. Withdrawal cards can only be issued to a member in good standing. Taking a withdrawal card from the Local will put you into INACTIVE status as a member. Crewmembers who have received a withdrawal card do not pay dues, pay assessments, participate in union meetings, or vote on union referendums (except contract ratification votes when a member is on furlough). By completing this form, you are requesting to be placed in inactive status as a member and will be issued a withdrawal card upon confirmation that you are a member in good standing. Upon your return to work, you are required to re-deposit your withdrawal card with the local to avoid a re-initiation fee.
